Biography/ Philosophy of Care
Hannah O. Jazdzewski, APRN, AGPCNP, is a nurse practitioner with Norton Community Medical Associates. She earned her master’s degree in nursing from Vanderbilt University School of Nursing in Nashville, Tennessee. She is certified as an adult-gerontology primary care nurse practitioner.
Hannah’s philosophy of care centers on building strong relationships with patients and their families, ensuring they feel heard, informed and involved in every decision. She is especially passionate about advanced care planning — helping patients define their goals and values so their health care decisions align with what matters most to them. Through open communication, education, and collaboration, she aims to promote autonomy, dignity and quality of life for older adults.
Outside of work, Hannah enjoys spending time outdoors with her husband, two dogs and chickens. Her favorite pastimes include running, hiking and working with horses.
